ÉVALUATIONS CLINIQUE ET ÉLECTROMYOGRAPHIQUE DE L'EFFET DE L'ACUPUNCTURE SUR LES PATIENTS SOUFFRANTS DE TORTICOLIS SPASMODIQUE

Résumé: Pour mesurer l'effet de
l'acupuncture chez des patients présentant un torticolis spasmodique (TS),
32 patients (17 hommes et 15 femmes) avec un âge moyen de 40,8 ans ont été
évalués cliniquement et par électromyographie (E.M.G.). Les évaluations sont
réalisées avant et après 10 séances d'acupuncture. Des aiguilles piquées dans
des points des méridiens pour des problèmes primaires (hypertonie ou hypotonie
ou mouvements non soutenus de la tête) et des aiguilles multiples épidermiques
(marteau fleur de prunier) sur des points sensibles pour des problèmes
secondaires (racourcissement musculaire ou cutané ou d'autres facteurs) étaient
appliqués de façon hebdomadaire. L'évaluation clinique montrait une
amélioration chez 23 patients (71,9 %) et pas de changement ou
détérioration chez 9 patients (28,1 %) après 10 séances d'acupuncture comparé à
avant traitement. L'évaluation E.M.G. montrait une amélioration chez tous
les patients après traitement par acupuncture. Il n'y eut aucun effet
indésirable chez aucun patient. Les caractéristiques de l'inefficacité de
l'acupuncture étaient les suivantes: 1) la longue durée de la maladie, 2) la
position du cou en extension, 3) la sévérité de l'incapacité à tenir la tête, 4)
un intervalle supérieur à une semaine entre deux traitements, 5) le patient
avait reçu différents types d'acupuncture. Ces évaluations suggèrent que
l'acupuncture utilisant les aiguilles pour les problèmes primaires et les
aiguilles épidermiques multiples pour les problèmes secondaires est un
progrès pour les patients présentant un torticolis
spasmodique.

Abstract : To investigate the effect of acupuncture in patients with spasmodic torticollis (ST), 32 ST patients (17 males and 15 females) with a mean age of 40,8 years were evaluated clinically and with electromyography (E.M,G.). Findings before and after 10 sessions of acupuncture treatment. The retaining needle on meridians points based on the meridian concept of primary problem ( hypertonus or hypotonus or unsustained head movement) and multiple epidermis penetreating needles on tender spots for secondary problem (muscle shortening or other factors) were applied weekly for acupuncture treatment. Clinical examination showed improvement in 23 patients (71,9 %) and no change and deterioration in 9 patients (28,1 %) after 10 sessions of acupuncture treatment compared with that before treatment. E.M.G. evaluation showed improvement in aIl patients following acupuncture treatment. There was no side effect of acupuncture treatment in any patient. The characteristics of ineffective patients by acupuncture was the following: 1) the longer duration of disease, 2) the neck posture demonstrated torticollis, 3) the severe unsustained head movement, 4) the more 1 week interval of acupuncture and 5) the patient was treated by different method of acupuncture. These findings suggest that acupuncture using the retaining needle for the primary problem and multiple epidermis penetrating needle for the secondary problem are more promoting for patients with spasmodic torticollis.

: Para medir
el efecto de la acupuntura en pacientes que presentaban torticolis
espasm6dico (TS), 32 pacientes (17 hombres y 15 mujeres) de 40,8 anos de edad
media han sida evaluados clînicamente y par elelectromiografia CE.M.G). Las
evaluaciones se han realizado antes y después de 10 sesiones de acupuntura.
Cada semana se aplicaban unas agujas pinchadas en pontas de los merimanas
para problemas primarios (hipertonîa 0 hipotonîa 0 movirnientos no
sostenidos de la cabeza) y unas agujas multiples epidérmicas (martillo de
siete puntas) sobre pontas sensibles para problemas secundarios
(acortamiento muscular 0 cutaneo u otros factores ). La evaluacion clînica
mostraba Ulla mejora en 23 pacientes (71,5)0/0) y ningtin cambio 0 deterioro en
9 pacientes (28,1%) después de 10 sesiones de acupuntura, comparado con antes
deI tratamiento. La evaluacion E.M.G. mostraba Ulla mejora en rodas los
pacientes después deI tratamiento con acupuntura. No
tuba ningtin efecto indeseable en ningtin paciente. Las
caracteristicas de la ineficacia de la acupuntura eran las siguientes: 1)
larga duracion de la enfermedad ; 2) la posicion deI cuello en extenl)ion ; 3)
la severidad de la incapacidad en mantener la cabeza ; 4) un intérvalo
superior a Ulla semana entre dos tratarnientos ; 5) el paciente tabla recibido
diferentes tiras de acupuntura
Estas
evaluaciones sugieren que la acupuntura que utiliza las agujas para los
problemas primarios y las agujas epidérmicas multiples para losproblemas
secundarios son un progreso para los pacientes con torticolis
espasm6dico.

The internet, by putting medical science with its fabulous bibliographic data
banks at the range of ail one each one, cannot improve the practice of any even
impregnated acupunctor of Chinese traditional Medicine. And without cutting this
thousand-year-old heritage, the acupunctor will be able to keep himself
informed of an omnipresent scientific topicality. But how not to be mislaid
in the gigantic labyrinth of the Internet and to find at the good place coveted
